This Falcon 2.35.0 EHR Module (Ambulatory) is Stage 2 –2014 Edition compliant and has been certified by an ONC-ACB in accordance with the applicable certification criteria adopted by the Secretary of the U.S. Department of Health and Human Services. This certification does not represent an endorsement by the U.S. Department of Health and Human Services or guarantee the receipt of incentive payments.

Falcon Physician offers a suite of nephrology-focused software tools, available within tiers of service. The Silver tier of service provides tools at no cost to DaVita-affiliated providers for clinical documentation in DaVita facilities. It is intended to be used in conjunction with a certified EHR product of the users’ choice. The Gold tier of service includes a la carte products for a monthly subscription fee. The Platinum tier of service includes access to all Falcon products, including the office EHR. Pricing for Falcon Platinum is as follows:

Falcon Platinum is licensed by licensed billing provider (LBP). Practices are charged a one-time implementation fee based on practice size that includes training and configuration of the product. Additionally, LBPs are charged ongoing monthly subscription fee. Additional fees will apply for optional data conversions as well as laboratory and/or practice management interfaces.

Third Party Services

Additional software available with Falcon Physician products include:

  • SureScripts Network for Clinical Interoperability to support receipt, display, creation, and transmission of Transition of Care/referral summaries to and from healthcare providers using the Direct protocol. This functionality for Falcon Physician products is only provided through the SureScripts Network.  There are no contractual or technical limitations of this service.
  • Health Companion as the patient portal to support view, download, and transmission of patient information to patients.

Contractual, Technical and Practical Limitations

Users are required to maintain hardware and network connection according to minimum system requirements.

Implementation scheduling and duration is dependent on practice size, complexity and Falcon availability.

Falcon is certified to the incorporate lab results (170.314.b.5) certification criteria. For the purposes of achieving MU, an EP may find that an electronic interface from laboratories will provide an automated mechanism to document lab results. Falcon (2.35.0) may require a one-time and/or on-going maintenance costs to establish interfaces to laboratories. Interface implementation scheduling and duration is dependent on practice size, complexity, Falcon availability and third-party coordination.

Falcon is certified to the view, download, and transmit to a 3rd party as mandated by 170.314.e.1 certification criteria. This certification criteria requires users to provide an online patient service or a “patient portal” to their patient’s.
